- A copy of the license for a licensed activity
- A document confirming the payment of license fee for the right to engage in certain practice (for those applying for the first time)(Data form)
- The application for obtaining the license in a form, (in electronic form)(Data form)
- A copy of waste purchase/sale contract (agreement) or other alienation agreement between participants of a foreign trade transaction
- Consent (in writing) of an authorized body in the field of environmental protection of the country into the territory of which hazardous waste is imported and (or) through the territory of which hazardous waste is moved in accordance with paragraph 4 of Article 6 of the Law of the Republic of Kazakhstan as of February 10, 2003 No. 389 "On the accession of the Republic of Kazakhstan to the Basel Convention on the Control of Transboundary Movements of Hazardous Wastes and Their Disposal "(hereinafter: the Basel Convention) (in case of hazardous waste export)
- A copy of contract (agreement) between an exporter and a manufacturer or an importer and a consumer of goods (if an applicant acts as an intermediary)
- A copy of contract(s) (agreement(s)) for transportation
- A copy of statement of the state environmental expert review
- A copy of contract between an exporter (importer) and a person in charge of waste disposal, which stipulates the environmentally safe use of the waste
- Notification on transboundary movement of hazardous wastes in accordance with paragraph 1 of Article 6 of the Basel Convention (in 3 (three) copies)
- Document on waste transportation in accordance with Appendix V to the Basel Convention (in 3 (three) copies)
- A copy of license to perform the type of activity involving use of waste
- A copy of document confirming coverage by insurance, pledge or other guarantee for the transboundary movement of hazardous wastes in accordance with paragraph 11 of Article 6 of the Basel Convention
- A copy of license to perform the type of activity involving use of waste
