- Application of a legal entity in the form of an electronic document certified by the electronic digital signature of the service recipient, in the form according to Annex 2 to this Regulation, or a statement of an individual in the form of an electronic document certified by the electronic digital signature of the service recipient, in the form according to Annex 3 to this Regulation(Data form)
- Information confirming the license fee to the budget, except in the case of payment through the "e-government" payment gateway(Data form)
- Electronic form of information on compliance of the service recipient with the qualification requirements according to the form according to Annex 4 to this Regulation
For individuals
