- Application of a legal entity in the form of an electronic document certified by the electronic digital signature of the service recipient, in the form according to Annex 2 to this Regulation, or a statement of an individual in the form of an electronic document certified by the electronic digital signature of the service recipient, in the form according to Annex 3 to this Regulation
- Information confirming payment of the license fee to the budget, except for the cases of payment through the "e-government" payment gateway(Data form)
- Electronic form of information on compliance of the service recipient with qualification requirements for performance of services on identification of technical channels of information leakage and special technical means, designed to conduct operational-search measures, according to the form according to Annex 4 to this Regulation or electronic form of information on compliance of the service recipient with qualification requirements for performance of services on identification of technical channels of information leakage and special technical means, designed to carry out operational-search measures, within the framework of the activities of the information security operational center or information security incident response service, in a form according to annex 5 to these Rules(Data form)
