- Application on issuance of a license formed in accordance with Instruction on filling out of application on issuance of the license for export and (or) import of separate kinds of goods and on formatting the license approved by Permission of Body of Eurasian economic commission from November 6, 2014 #199(Data form)
- Form of data on compliance to qualification requirements(Data form)
