- Electronic copy of application in a form (necessarily )(Data form)
- Copies of the documents confirming existence of the simulation equipment, equipment and medical tools (necessarily )
- Copies of documents confirming personnel information of the evaluation organization (necessarily )
- A document certifying a proprietary right or lease contract or immovable property gratuitous use contract or fiduciary management contract or public-private partnership contract, in case if there is no data in the e-Gov Gateway IS (necessarily )
- Documents certifying an employee`s labour experience in accordance with subtypes of activities pursuant to Article 35 of the Labour Code of the Republic of Kazakhstan, in case if there is no data on employees in the e-Gov Gateway IS (necessarily )
- Assessment organizations drawing up a methodology: strategic development plan, in-house documents regulating the procedure for assessment of knowledge and skills aimed at students, graduates of vocational organizations in the field of healthcare (necessarily )
