- An electronic copy of sanitary and epidemiological statement in which a service recipient plans to handle pathogenic biological agents (necessarily , job specifications )
- An electronic copy of information on compliance with the qualification requirements for handling of pathogenic biological agents approved in accordance with subclause 40-5) of Article 9 of the Code of the Republic of Kazakhstan "On the Health of People and Healthcare System" (hereinafter: qualification requirements), depending on the claimed research nomenclature (necessarily , job specifications )
- An electronic copy of explanatory note indicating: - nomenclature of research in the form according to Appendix 7 to the rules; - flow patterns of the material (necessarily , job specifications )
- An electronic copy of certificate on certification course for treatment of pathogenic biological agents (necessarily , job specifications )
