- Information on registration of ensuring the performance of duties of a legal entity carrying out activities in the field of customs (necessarily )
- Notarized copies of documents confirming the ownership, economic management, operational management or lease of vehicles used for the transport of goods, including vehicles suitable for the transport of goods under customs seals and stamps, which are supposed to be used in carrying out activities as a customs carrier (necessarily )
- Copies of certificates of approval of vehicles of international transport for the transport of goods under customs seals and stamps (necessarily )
- A copy of the authorization document for carrying out cargo transportation activities, if such an activity requires the presence of the specified document (necessarily )
- Request in the form of electronic document identified by e-digital signature of service receiver (necessarily )(Data form)
