- Application in the form of electronic document certified by e-digital signature of service receiver (formed electronically)(Data form)
- Explanatory note with a list of works proposed by the applicant, carried out at the landfill - in arbitrary form, explanatory note should be signed by the applicant or licensee
- The radiation safety instruction approved by the applicant during the stated work, which meets the requirements of the Sanitary and Epidemiological Requirements for Radiation Safety specified in Appendix 6 to the Sanitary Rules, approved by Order of the Minister of Health of the Republic of Kazakhstan No. RK ДСМ-275/2020 dated December 15, 2020 (registered in the Register of State Registration of Regulatory Legal Acts No. 21822)
- Radiation monitoring plan (in case of presence of own radiation monitoring service)
- Radiation control device check certificates (in case of availability of private radiation control service)
- Radiation Safety Service Regulation (or job instructions of the responsible person for radiation safety)
- A form of information containing information on providing personnel with individual dosimetric control (Data form)
- A form of information containing information about the service or person in charge(Data form)
- A form of information containing information about the qualified composition of specialists, technicians and workers(Data form)
- Form of information containing information on the contract for the provision of services for industrial radiation control at the site of work
- Information confirming payment for the provision of public service through the payment gateway of "e-government"(Data form)
